Complete and Complete Plus - coverage is only available in a 60 day open window.

Optimum - if you choose to apply for the Optimum level of coverage (complete a Form 2, Statement of Health). This is totally optional and your acceptance into this plan is based on your medical history. Optimum is offered at the same cost as Complete. During the underwriting process for Optimum, you will be enrolled in the guaranteed option of your choice. Rates will be adjusted if applicable at the time of approval.

If you are not approved for the Optimum this will not impact the guaranteed options available to you.

Stated maximums are per benefit year, unless otherwise specified and apply to each plan member and insured dependent.
